Endometriosis Concerns: How to Describe Your Pain

When pelvic pain is difficult to explain, a short diary can help you describe it more clearly. Pain that disrupts school, work, sleep or relationships deserves a conversation with a healthcare professional. A diary supports that conversation; it does not confirm endometriosis or rule out another cause.

Endometriosis may cause painful periods, pelvic pain and other symptoms, but experiences differ. Tell the clinician about the full pattern rather than trying to make your symptoms fit a particular diagnosis.

Describe the experience in your own words

Record where the pain is, when it happens and how long it lasts. Words such as aching, cramping, sharp or pressure may help, but there is no required vocabulary. If you use a pain score, also explain what the pain stops you doing.

  • Does it occur before, during or between periods?
  • Is there a relationship to sex, passing urine or opening your bowels?
  • Does it affect sleep, movement or your usual responsibilities?
  • What have you tried, and what effect did it have?
  • Are there other symptoms, bleeding changes or pregnancy concerns to mention?

A useful entry might be: “Tuesday evening: lower pelvic pain for two hours; missed my usual walk; started the day before bleeding.” This is more informative than a score alone.

Bring the previous investigation reports

If you have had scans, surgery or blood tests, bring the reports if available. Include earlier treatment names and explain whether you stopped them, why and on whose advice. Tell the clinician about your fertility goals because these can affect the discussion of treatment choices.

Ask what the likely next step is, what an investigation could show and what it cannot reliably exclude. The assessment may involve more than one appointment.

Do not wait for a complete diary if you need help

Sudden severe pain, fainting or pain with possible pregnancy needs urgent assessment. For continuing symptoms, make a routine appointment without waiting for several months of notes.
